How To Turn in a Physical
Need to turn in a physical?  Here is what the athlete needs to do.  Athletes can take their physicals to the Athletic Trainer, Mr. Allen, during lunch or after school.  Once the Trainer checks the form and makes sure it is complete, the athlete will be given a slip to give to his/her coach.  This slip lets the coach know the athlete can now participate in tryouts.  Please do not give your physical to a coach, the front desk, or put in the Athletic Trainer's mailbox.  This must be turned in before an athlete is allowed to participate, no exceptions.  Please plan ahead.

Tryout Forms

An APS Physical and Concussion Management Course certificate are required to participate in tryouts. Complete and return the forms to your coach by their specified due dates.
