The above link allows you to enter information that is needed for your athlete.  Please complete the steps below before clicking on the link.  Completing these steps will make filling out the online form go quicker.  The link will open up again at the start of the 2021-22 school year.  Athletes can complete the below requirements at any time.

Mandatory Meetings

Every school year athletes and a parent must attend the NMAA required mandatory meeting.  This meeting only needs to be attended once every school year.  All meetings will start at 6:00pm and will be in the PAC.  Dates of this meeting are:

August 16, 17, and 18                                          November 22                                                           February 14

How To Turn In A Physical

IF AN ATHLETE IS PARTICIPATING IN SUMMER ACTIVITIES:  The coach of that sport will pick up and check physical forms.  When the athletic trainer returns from summer break, the athlete will be given back the physical to take to the trainer and receive a blue slip.  PLEASE MAKE SURE ALL FORMS ARE INCLUDED AND FILLED OUT.

In order for students to participate in athletes all athletes need to have a physical dated after April 1, 2021.  When paperwork is completed, you can call the athletic trainer, Mr Allen, to set up an appointment to turn the forms in.  He can be reached at 505-273-0845.

Tryout Forms

An APS Physical and Concussion Management Course certificate are required to participate in tryouts. Complete and return the forms to the athletic trainer before practice begins.
